New Heart Failure Resources
After undertaking some consumer research, the Heart Foundation has updated its heart failure resources creating:
- a new heart failure action plan,
- a new resource for people supporting whānau with heart failure
- a pack that pulls our resources together in one place.
There’s a new heart failure hub on their website, which includes videos on heart failure management and personal stories. Arthur’s is a great example of the personal stories that can be shared with patients and their whānau.
These resources are designed to be given to patients at the point of diagnosis and to those living with heart failure, with the aim of supporting their heart failure self-management in conjunction with their healthcare professionals. There is also a resource for whānau designed to support them in the care of people with heart failure.
